We offer services for women, men, children and infants. Each patient's treatment plan is individually designed after a thorough initial evaluation.
Required for Physical Therapy Treatment
- You must have a written referral for Physical Therapy treatment from a qualified healthcare practitioner (physician, physician assistant, nurse practitioner, dentist, podiatrist or chiropractor). The referral needs to include your diagnosis, the words "Evaluate and Treat," and be signed by the healthcare practitioner who referred you.
- Once you have a written referral, you may call us to schedule your appointment. When you call, we will ask you for some basic information, including your insurance company. We are able to schedule most patients within 48 hours from the time you call.
- Prior to your first visit, please print and fill out the patient forms available below.

What to expect on your first visit
- Arrive 15 minutes early with your paperwork completed, see patient forms below.
- We will ask for your written referral, driver's license and insurance card, which we will make a copy of for your file.
- You will be seen by one of our Physical Therapists for a comprehensive initial evaluation. During the evaluation they will review the following:
- Your current problem/diagnosis
- Your medical history
- Your pain level and how it is affecting your daily life
- Your physical therapy goals
- Your physical therapy treatment plan
- Allow 1 hour for your Initial Evaluation.
- We request notification of cancellation at least 24 hours in advance.
